On Sunday, 28.04.2013, at the final stage of surgery during the preparation for implantation IOL one of our clients “Vash Semeiniy Doctor” clinic (in Yekaterinburg) opened the container with IOL (SN6AD1 +24,0 D,  series # 121 65053 011 ). There was discovered the damage of container including haptic (around the middle of haptic) in the form of crush injury.  While attempting to extract IOL from the container, distal part of haptic separated from. IOL was not implanted, patient left with aphacia (without IOL, with empty capsule bag). In consequence of the patient was operated on at 30.04.2013, and he was implanted the other OIL SN6AD1 +24,0 D. In view of the event, make the return IOL (SN6AD1 +24,0 D), please.
