Pharmacology


The compound is applied to treat bacterial infections; the bacterial and static activity of  josamycin being the macrolide is connected with inhibition of protein synthesis in bacteria. The compound being the antibacterial preparation is applied in case of highly inflammatory infections.


 


Josamycin is a highly active compound against intracell microorganisms as well as the one against some anaerobic bacteria. This preparation rarely affects enterobacteria. So it may rarely affect the natural bacterial flora of the intestine. The compound is effective in case of resistance to erythromycin. The resistance of josamycin is rarely occurred if compared with other antibiotics of macrolide group.  


 


Pharmacokinetics


 


If the preparation is applied per os, it can quickly be found in the intestine, inter alia, the preparation does not affect the biological availability. The maximum concentration of josamycin can be found in the serum one or two hours after the preparation has been applied. Near 15 per cent of the preparation is tied by the proteins of the plasma. Very high concentrations of the preparation  can be found in the lungs, tonsils,  saliva, sweat and lachrymal fluid. The concentration of the preparation found in sputum is 8-9 times higher than the one in plasma. The preparation  can also be accumulated in bone tissue. It passes through placenta, and is secreted during lactation. Josamycin is metabolized in the liver and disintegrated into less active metabolites. It excretes basically with the bile and about 20 per cent or less is excreted with the urine.


 


Application and dose    


 


The medicine shall be daily applied 1-2 grams for the grownups and teens older than 14 years. If needed it can be applied in a dose of 3 grams per day. The daily dose shall be divided into two or three applications.


 


The daily dose for children is applied 20 - 50 mg/kg of the body weight and should be divided into 2 or 3 applications.


 


In case of Acne vulgaris or Acne conglobata the medicine should be applied in a dose of 500 mg two times a day within 2-4 weeks, then it should be applied in a dose of 500 mg of the josamycin but only one time a day as a sustaining treatment within 8 weeks.


 


Dispersible tablets of Wilprafen® solutab can be applied in two different ways: the medicine can be swallowed as a whole with a drinking water or being dispersed in water beforehand. The tabs should be dispersed in at least 20 mg of drinking water. But before the medicine is applied this should be stirred in a cup of water. Normally, the duration of application period shall be determined by the medical doctor. According to World Health Organization prescription to treat against Streptococcum infections the duration period shall be not less than 10 days.
